Youth Case Management

Youth case managers are an active part of the TCN treatment team of professionals working to stabilize high-risk youth clients in school and community settings. Youth case managers provide linkage and referral to community resources and are directly involved in the coordination of care for youth clients in need of psychiatric hospitalization.

School navigators are primarily located in the schools and work closely with school building staff to foster a nurturing school environment. School navigators provide consultation and support to students, families and school staff regarding programming and intervention to address behavioral concerns of students. Like youth case managers, school navigators are able to provide linkage and referral to community resources.
